UPDATE: The newest version of Combo Form is to replace ALL previous Work Order & Fee Schedules
including version 4 Combo Form, effective immediately. The necessary change was to update USPS Priority Mail fee from $8 to $10. We have no control over third party service costs.
Transactions to email: Membership (New, Renewal, Youth, 4H & Supporter); Registrations (Litter, New, Provisional, Recovery) ; Duplicate Certificates, Fundraiser Contributions, Research and more.
Send Paypal receipt or confirmation along with Combo Form, appropriate documents and photos
by email to Registry (asregistry@gmail.com).
Transactions to mail: Transfers of registered stock, Registrations or Transfers with Bill of Sale.
Wet ink signature documents include: Certificate of Registry for Transfer of ownership and
correction to record, and Bill of Sale. Seller and buyer complete the reverse of Certificate of Registry. Retain copies of front and back of Certificate of Registry and Bill of Sale. Enclose photos of ALL Transfer stock and Transfers with Bill of Sale. Alternatively, email photos within 3 days to asregistry@gmail.com, with Farm name, the word “Transfer” and Animal ID (name, tag # or Certificate of Registry number) in the subject line.
Failure to submit photos in a timely manner will delay processing.

Bill of Sale original with application packet MUST be submitted for acceptance & approval to:
AMBA, PO Box 2883, Appomattox, VA 24553
AMBA Main office will forward to Registry at no additional charge
Documents improperly submitted and/or approved or processed in error are subject to review and removal.
It is the responsibility of Breeders, Buyers and Sellers to make sure documents are complete and correct.

PayPal users should complete documents before payment,
and submit email as attachments to asregistry@gmail.com, including
PayPal confirmation receipt with “AMBA”, transaction type,
and your name or farm name in subject line.
Checks should be mailed with documents needed
(Combo App, Registration Application, Bill of Sale, if required) to:

Pig & Hog Photographs are required of all Registration forms
relating to pigs and our Herd Book.
Email photos to: mulefootregistry@gmail.com, and
put your farm name/member name and animal ID in the subject line. This should be done as soon as you submit documentation. .

Best to email them first so we can make sure all points needed are clear enough to place them correctly. (Provisional and Recovery pig owners already comply.)
