APPLICATIONS and PHOTO INSTRUCTIONS

2 - Registration Application Form (Required for all Herd Book work)
3 - Photographs Required for Registration/Transfers (Part of our DNA & Diversity Program)
**Bill of Sale MUST have wet ink signatures and be mailed to Main Office. Original will be returned.
***AMHA&R (American Mulefoot Hog Association & Registry) registration certificate originals
MUST be mailed to Main Office for AMBA registrations. Original will be returned.
PHOTOS REQUIRED FOR ALL REGISTRATIONS, TRANSFERS AND BILL OF SALE PROCESSING.
INSTRUCTIONS for submitting your Membership or Registration:
The first document required for new members or yearly renewal of membership is the
ComboForm v7 (Membership Application & Fee Schedule).
You can submit an updated contact information thru the ComboForm v7, or just drop us an email.
Email to MulefootRegistry@gmail.com, subject line: Membership with last name or farm name
Mail to: AMBA-Membership, PO Box 2883, Appomattox, VA 24522-2883.
Documents required for Registration of New Stock or Litters are:
Registration/Litter Registration Application and ComboForm v7.
If there is incomplete information or a lag of time from ownership transfer,
both buyer and seller must complete and submit an original Bill of Sale.
This form must be “wet signature” only, witnessed,
and original document mailed to AMBA - Registry.
The Bill of Sale is also useful in "unpapered" purebred stock,
Recovery Program candidates, in unregistered from registered stock, or Qualifier (no papertrail) candidates.
Original documents will be returned.

**Note: ORIGINAL documents from Transfers or Bills of Sale transaction
MUST be submitted to the Registry Office before work will begin.
All animal transfers and registrations will only be completed for Members whose dues are current for the year.
We no longer accept: Non-Member Registration for animal registrations or transfers.
PLEASE keep clear copies of front and back of any original work submitted to Registry Office.
It may be necessary for you to email scanned copies prior to applying for registration for pre-approval.
